We will handle any type of investment permitted by law and will work with self-trusted plans as well as any custodian, trust company, broker or bank.

Plan Document:

  • Full compliant prototype document is completed and is signature ready

Services:

  • Census Analysis. We can provide services in reviewing participant data for eligibility, service, vesting and more.
  • Vesting Administration. Quarterly review of service and hours for processing and reporting.
  • Participant Loan Administration. Participant level loan processing, administration and ongoing maintenance.
  • Participant Withdrawal and Distribution Administration. Participant level processing of hardship and in-service withdrawals in addition to termination distribution requests.
  • Annual Discrimination Testing. Full annual testing package and year-end allocation or refund processing.
  • Annual Form 5500 filing. Signature-ready government filing with attachments and Summary Annual Report. We'll work with the plan's accountant and/or auditors to make the annual audit more efficient.

Reporting:

  • Quarterly participant statements - standard or customized
  • Quarterly plan sponsor valuation report
  • Reconciliation of trust report

Tax Forms:

  • Form 5500 (appropriate form of this series depending on plan size)
    • With EFast Electronic Filing
  • 1099 Reporting

Instructions for opening a New 401(k) or other defined contribution plan

Print out the New 401(k) Plan Set-up Form and complete it. Feel free to call us toll-free at 877-254-7085 if you have any questions while completing the set-up form.

The completed New 401(k) Plan Set-up Form along with a check for the one-time installation fee of $250 should be mailed to:

Once the Revzon Consulting Group has received the completed New 401(k) Plan Set-up Form along with a check for the fee we will begin to process your plan. It will take approximately two (2) weeks to set up the plan and produce the plan documents along with the ancillary forms.

When you receive the plan documentation package it will contain detailed instructions as to which forms need to be signed, which ones need to be returned to us, which ones you should keep, and which ones need to be given to employees.

After signing and returning the appropriate documents that we have sent to you, the plan is officially established. We will work with your financial services firm regarding the asset side of the plan to ensure we are able to track all of the plan assets.

Instructions for opening an Existing 401(k) or other defined contribution plan

Obtain a copy of the current plan document [this could be an individually drafted plan or a prototype plan and adoption agreement]

Obtain copies of the last two years of 5500 forms

Obtain copies of the last two years of testing data and reports

Obtain copy of the current employee census

The completed Conversion 401(k) Plan Set-up Form, current plan document, 5500 forms, testing data/reports, and employee census along with a check for the one-time installation and conversion plan fee of $700 should be mailed to:
